Fantasy Fest הוא האירוע המרהיב ביותר של קי ווסט ואחד הציבוריים יוצאי הדופן בארה"ב — פסטיבל בן עשרה ימים מדי אוקטובר מאז 1979, שמהפך את האי לקרנבל של תחפושות, ציורי גוף, מסיבות רחוב נושאיות וחגיגה ציבורית חסרת עכבות. הפסטיבל מושך מעל 75,000 מבקרים לאי של 25,000 תושבי קבע, וממלא כל מלון, אכסניה ונכס להשכרה באי. לפסטיבל יש השתתפות להט"בית עמוקה מובנית ב-DNA שלו: החגיגה של תחפושת, הסובלנות לעירום ציבורי וצבעי גוף, תרבות מסיבות הרחוב והחיבוק המפורש של חופש מיני וזהות הופכים אותו לאירוע גיי טבעי גם כשהוא מושך חתך רחב של תרבות המסיבות של אמריקה. תחרות התחפושות Pretenders in Paradise היא אירוע היום החתום של הפסטיבל. צעדת Masquerade בשבת האחרונה מושכת כ-75,000 צופים לאורך Duval Street. ה-Painted Ladies — נשים בציורי גוף שמטיילות ברחובות העיר העתיקה — הן הדימוי האייקוני של Fantasy Fest. האירוע דורש הזמנת לינה שמונה עד עשרה חודשים מראש.
