Fantasy Fest è l'evento più spettacolare di Key West e una delle celebrazioni pubbliche più straordinarie degli Stati Uniti — un festival di dieci giorni di ottobre, attivo dal 1979, che trasforma l'isola in un carnevale di costumi, body painting, feste di strada a tema e celebrazione pubblica senza freni. Attira oltre 75.000 visitatori su un'isola con 25.000 residenti fissi, riempiendo ogni hotel, guesthouse e affitto. Fantasy Fest ha una partecipazione LGBTQ+ scritta nel suo DNA: la celebrazione del costume, la tolleranza per la nudità pubblica e il body paint, la cultura della festa di strada e l'esplicito abbraccio della libertà sessuale e dell'identità performativa ne fanno un evento naturalmente gay anche se richiama un pubblico ampio della cultura festiva americana. Il concorso Pretenders in Paradise è l'evento diurno simbolo. La Masquerade March street parade del sabato finale richiama circa 75.000 spettatori lungo Duval Street. Le Painted Ladies con il corpo dipinto che camminano per Old Town sono l'immagine iconica del festival.
