Las Vegas has two gay bathhouses, both open around the clock and both a short ride east of the Strip. It's a small scene, but a real one — these are established venues, not places we've padded out to fill a page.
Adonis Bathhouse Gay Men's Health Club
The largest of the two, Adonis sits on East Flamingo Road and runs 24 hours with a gym, dry sauna, steam room, private rooms and a pool. It draws a mix of locals, tourists and convention visitors, and it's busiest on weekends and event weekends.

Entourage Vegas Spa and Health Club
Operating since 1994 in the Commercial Center (the former Apollo Spa), Entourage is a 24-hour men's bathhouse with a pool, sauna, steam and private cabins. It's the older of the city's two spas and keeps a steady local following.

Practical Notes
Both venues are men-only, 24/7, and charge for a room or a locker at the door (roughly $20-35 depending on the option and time of day). Towels and lockers are provided. As with any bathhouse, discretion is expected and Nevada's public-decency laws apply.
